新しい研究: 異なるFADS遺伝子型を持つ乳児にARAとDHAを補充した乳児用調合乳を与えた場合の脂肪酸レベルに対する影響

母乳は満期産児に与える栄養のゴールドスタンダードであり、母乳代用製品の組成の基準として使用されています。乳児用栄養製品は可能な限り厳密に母乳の組成と機能性を模倣するように設計されています。母乳中には常にアラキドン酸(ARA)とドコサヘキサエン酸(DHA)が含まれています。これらの脂肪酸は、脳や眼の発達と機能、および幼少期における免疫機能に重要な役割を果たします(2-4)。  

脂肪酸デサチュラーゼ(脂肪酸不飽和化酵素)遺伝子(FADS)

DHAとARAは必須脂肪酸であるα-リノレン酸(ALA)とリノール酸(LA)からそれぞれ体内で合成されますが、内因性合成は限られており、遺伝的背景の影響を受けるため、母乳で育てられた乳児により近い状態を達成するためには、事前に形成されたDHAとARA供給源が必要です(5,6)。  脂肪酸デサチュラーゼ遺伝子FADS 1とFADS 2遺伝子はそれぞれD5およびD6デサチュラーゼ酵素をコードし、これはエロンガーゼ酵素と一緒に働き、それぞれALAとLAをDHAとARAに変換します。したがって、D5およびD6デサチュラーゼ酵素は変換プロセスにおける律速段階と考えられます(7)。

DHAとARAに対するFADS遺伝子多型の影響

FADS遺伝子中の一塩基多型(SNPs)は、D5およびD6デサチュラーゼ酵素活性を低下させ、DHAおよびARA合成を低下させることが示されています(8-10)。マイナーFADS遺伝子多型は、メジャーFADS対立遺伝子よりも低いデサチュラーゼ活性を示します(対立遺伝子は別の形態の遺伝子です)。FADS遺伝子多型によって、ARA血中濃度は最大で28%変動する影響を受けます(11)。このような遺伝子変異に関連するDHAとARAの合成低下はEU人口の約30%で報告されており、アジアやメキシコではさらに高い可能性があります(11-13)。さらに複数の研究で、FADS遺伝子多型が脂肪酸の状態に及ぼす影響、およびそれに関連する知能の発達、免疫機能、小児期のアレルギーのリスクについて言及があります(14)。

COGNIS研究

COGNIS研究では、健康な乳児のための新しい乳児用調合乳の神経認知的および免疫学的な影響を調査しました。さらに、これらの乳児のサブセットにおいて、研究者は特に、乳児用調合乳を与えた乳児と母乳栄養乳児の長鎖多価不飽和脂肪酸(LCPUFA)レベルへの食事およびFADS遺伝子多型の影響を調べました(1)。ここでは6か月齢未満の176名のスペイン人乳児を評価しました。乳児は2種類の乳児用調合乳グループのどちらかに無作為割付されました。標準的な乳児用調合乳(SF)にはDHAとARAは含まれず(n=61)、新しい乳児用調合乳(EF)には100 mlあたり11.2 mgのDHA(16mg DHA /100 kcal)と、15.8 mgのARAが含まれていました(23mg ARA/100 kcal) (n=70)* (15)。どちらの乳児用調合乳とも、欧州小児消化器学・肝臓学・栄養学会(European Society for Paediatric Gastroenterology, Hepatology and Nutrition, ESPGHAN)の栄養委員会ガイドラインおよび乳児用調合乳に関する国際および国内の勧告に従っています。参照グループは母乳栄養乳児(n=45)でした。3か月齢で採取した頬細胞から、脂肪酸レベルとFADS遺伝子多型を分析しました。

FADS遺伝子多型対立遺伝子ごとに乳児を分類した場合、一部の哺乳グループでは統計的に異なるDHAとARAの細胞レベルが報告されました。  母乳栄養乳児では、FADSメジャー対立遺伝子とマイナー対立遺伝子の保有者の間でDHAとARAのレベルに違いはありませんでした。論文著者は、母乳中に存在する高いDHAとARA濃度により、FADS遺伝子多型に関連するD5とD6デサチュラーゼ酵素活性の低下を克服した可能性があると考えています。実際にこれは、17 mg/100 kcalのDHAと0 mg、25 mg、または34 mg/100 kcalのARAの乳児用調合乳を無作為割付して乳児に与えた(34 mg/100 kcal はCOGNIS研究よりも高い濃度です)Miklavcic et alによる研究でも認められました(16)。  FADSマイナー対立遺伝子保有者では、血漿中のARAは最高レベルのARA補充でのみ、0 ARAグループより高くなりました。

対照的に、Salas-Lorenzo et alは、新しい乳児用調合乳グループ(EF)のFADSマイナー対立遺伝子保有者は、母乳栄養乳児グループよりも有意に低いARAレベルであったと報告しました。  つまり、マイナーFADS対立遺伝子は母乳栄養乳児のARA状態に影響しなかったが、これらの対立遺伝子の存在は、栄養補充された乳児用調合乳を与えられた乳児のARA状態を低下させました。驚くべきことに、標準乳児用調合乳グループ(DHAとARAの補充なし)では、乳児のARAレベルに違いはありませんでした。EFグループのマイナー対立遺伝子保有者の細胞内DHAレベルもまた、母乳栄養乳児グループよりも低く、標準乳児用調合乳グループよりは高いものでした。

EFグループのFADSメジャー対立遺伝子保有者は、デサチュラーゼ活性の低下を示さず、これらの乳児のDHAとARAのレベルは母乳栄養乳児グループと統計的な違いはありませんでした。しかし、SFグループのレベルよりも有意に高いものでした。  

遺伝子型に関係なく、この研究ではDHAとARAレベルはSF< EF< BFの順となりました。これらの結果からは、低いDHAとARA状態に脆弱である可能性のあるFADSマイナー対立遺伝子保有者の乳児グループでは、母乳栄養乳児により近い状態を達成するためには、高いレベルのDHAとARA補充が必要な可能性があると著者は結論しています。

*乳児用調合乳およびフォローアップミルクについての2015年9月25日付欧州委員会委任規則(EU) 2016/127では、20 – 50 mg DHA/100 kcalの添加を義務付け、ARAの添加については任意です。このEU規制は2020年2月から義務化されます。
