乳児の健康と発育に対する栄養の重要性についての最新の科学調査

先日、NUTRI-FACTSはフランスのベルサイユで開催された、乳児と小児の健康に関する研究と科学の発展を報告する大きな小児科の大会である第20回Journées Interactives de Réalités Pédiatriques (JIRP)に参加してきました。女性の妊娠開始から子供の2歳の誕生日までの最初の1000日間の良好な栄養が、このイベントの重要なテーマでした。長鎖多価不飽和脂肪酸(LCPUFA)の中でも、特にドコサヘキサエン酸(DHA)とアラキドン酸(ARA)を含む食事性脂肪の重要性が注目されました。

乳児の発育に必須のDHAとARA

DHAとARAは母乳中に天然に存在する主要なオメガ3およびオメガ6長鎖多価不飽和脂肪酸であり、妊娠中は胎盤を介して優先的に乳児に渡されています。JIRPで発表された研究では、母乳中のDHAレベルは母親の食事によって変動する一方、ARAレベルはより一定であることが示されました。(1)  母乳中の一定のARAレベルは、これが乳児の健康と発育に重要な役割があることを示唆しています。最近の研究では、生後12か月までの間、DHAとARAを一緒に補充すると、9歳までの脳の構造と機能をサポートするなど、子供の認知発達に良い影響を与える可能性があります。(2,3)  さらに、過去20年にわたる研究によって、DHAとARAが乳児期の視覚の発達と機能、血流、免疫システムの発達と機能に果たす重要な役割に注目が集まっています。(4-6)  DHAとARA脂肪酸は乳児の成長と発育に明白に重要であり、母乳で育てることができない場合には、DHAとARAの両方を補充した乳児用調合乳を乳児に与えることが推奨されています。

乳児向けの乳児用調合乳に関するEUの新たな規制

この証拠にもかかわらず、最近のEU規制の変更では、乳児向けの乳児用調合乳の成分にDHAの添加を義務付けている一方、ARAの添加は製造メーカーの裁量に任されています。(7)  乳児用調合乳についての2015年9月25日付欧州委員会委任規則(EU) 2016/127では、乳児の健康と発育のためには、最低20 mgのDHA/ 100 kcalの添加が必須となっている一方で、ARAは任意の成分のままです。(8)  これらの法的な指定は2020年2月以降は義務化されますが、複数の科学者からは、意図しない結果を招く可能性があるとして懸念が指摘されています。科学者たちは、ARAなしで新たに義務付けられたDHAレベルの製品への補充が、乳児の成長と発育をサポートするために十分であるかどうかを決定するためのさらなる研究が必要であると述べています。(9-12)  乳児の脆弱性を考慮して、専門家たちは研究の欠如に懸念を表明しました。NUTRI-FACTSは母乳の組成とは異なる栄養製品を乳児に与えることは時期尚早である可能性があるとの結論に達しました。そのため、ステージ1乳児用調合乳では、DHA単独ではなく、DHAに加えてARAを含めることを検討する必要があります。

乳児用調合乳で、DHAとARAのバランスを確保する

最適な乳児の発育と健康のためには、DHAとARAの状態、および脂肪酸レベルのバランスを考慮する必要があります。(13)  この背景にある科学研究がJIRPで説明され、オメガ3とオメガ6脂肪酸ファミリーは、食物から摂取される前駆物質をDHAやARAに変換する同じ酵素について競合しています。オメガ3脂肪酸と比較してオメガ6脂肪酸が多すぎるとDHA合成が制限されることがあり、一方でオメガ6脂肪酸と比較してオメガ3脂肪酸が多すぎるとARA合成が制限されることがあります。さらに、DHAやARA合成に関わるエロンガーゼ(脂肪酸伸長酵素)またはデサチュラーゼ(脂肪酸不飽和化酵素)の遺伝子多型は、正常な酵素活性に影響し、乳児における低レベル脂肪酸につながることがあります。新しい研究では、これらの遺伝子多型の負の影響によって、高いレベルのDHAとARAを補う必要の可能性のある乳児グループを特定しました。(14,15)

この研究を考慮して、国の規制では、DHAとARAが1:1~1:2の比率の栄養補充乳児用調合乳を許可しています。(16,17)  これは母乳で報告されているバランスを提供するだけでなく、幅広い研究で示されている最適な乳児の成長と発育もサポートします。しかし、DHAが豊富なイワシ、鮭、サバなどの脂の多い魚を食べないベジタリアンの母親では、脂肪酸の摂取が不足し、母乳中のDHAレベルが低くなるリスクがあります。したがって、母乳に含まれるDHAの量が最適になるように、週に1-2回脂の多い魚を食べるか、代わりに藻類サプリメントや魚油ベースのサプリメントなどを意識して摂取することが推奨されます。

乳児栄養ソリューションの将来

新しい研究では、母乳中に含まれているが、現在の乳児用栄養製品には含まれていない栄養素および生理活性物質の特定が続けられています。この証拠に基づいて、乳児用栄養製品に用いられる成分は、人生の重要な段階での成長と発達をサポートするために進化を続けるでしょう。
